Referral


At Passionate Support Services, we will accept referrals from relevant Community Mental Health Team(s) and Crisis Resolution/ Home Treatment Team.


Referrals will come from the following;

Directly from their home,

From A & E following psycho-social assessment and risk assessment (subject to PSS internal assessment and approval)

From Acute Admission Ward (following an MDT review and where deemed suitable for step down accommodation)

Under Community Order and/ or Mental Health Act - subject to assessment.


Tulip House will generally accept patients who meet the criteria for CRHTT support or regular support from their allocated care coordinator (Care Pathway).


Service users accepted may have psycho-social problems and those at risk of further deterioration in mental state. All service users will be expected to be willing to engage with allocated key worker staff at Passionate Support Services and other external teams such as local CMHTs and other support groups.
